How to create a request

• Only use for DCOM Maintenance Requests!
• Enter in “Request” Example: Faucet leaking
• Choose “Building” then “Location”
• Enter your cell phone number & Name in case we need to contact you.
• Describe in as much detail as possible what the issue is.
• Add any attachments if needed.
• Click “Submit”
